Notes:
# The country of Czechoslovakia was dissolved in 1992 forming two new countries - the Czech Republic (Czechia) and Slovakia. A combined team continued to compete through to the end of the 1994 World Cup qualifiers under the name of RCS (Representation of Czechs and Slovaks). The two nations have since competed completely separately.

Between 1903-1908, while part of the Austro-Hungarian Empire, Bohemia (sometimes referred to as Bohemia and Moravia) played a number of matches, making it the world's ninth oldest international team. An independant Czechoslovakia team played its first matches at the 1920 Olympics. The name Bohemia was resurrected while under Nazi occupation in 1939, but these matches are not regarded as official internationals.
